Lee Family Practice recognized nationally


LEE -- Lee Family Practice, the newest site of Community Health Programs, has been recognized as a "Patient-Centered Medical Home," by the National Committee of Quality Assurance.

A PCMH is a way of organizing primary medical care that emphasizes care coordination and communication. The goal is to create a more effective relationship between the providers and the patients.

"This is a major achievement for Lee Family Practice, CHP, and the community which is due in large part to the vision of Dr. Michael Kaplan and the hard work and of our providers and staff," said CHP’s CEO Bryan Ayars.

CHP is a nonprofit federally qualified health center providing primary care, dental services, and family services to residents of Berkshire County, with sites in Great Barrington, Lee, and Pittsfield.
