BENNINGTON, Vt. — Each of Southwestern Vermont Medical Center’s five primary care practices in the state of Vermont have been re-designated as Patient Centered Medical Homes by the National Committee for Quality Assurance. Two of the practices are located in Bennington, while the others are located in Manchester, Pownal and Wilmington.

To receive recognition, a practice must demonstrate its ability to meet six key standards of the Patient Centered Medical Home model, which was developed by the American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics, and the American Osteopathic Association.

NCQA is a private, non-profit organization dedicated to improving health care quality that accredits and certifies a wide range of healthcare organizations.
